🇱🇰 1,489 Doctors Left 2022-2024, Costing Rs. 12.5B for tax payers - New reasearch finds

⭕ 1,489 doctors, including specialists, migrated from 2022-2024 due to economic crisis.

⭕ Estimated financial loss to taxpayers: Rs. 12.5B (US$41.5M).

⭕ 🇱🇰 Sri Lanka's healthcare workforce: ~23,000 doctors, incl. 2,800 specialists.

⭕ 1,085 doctors on overseas specialist training; 205 (20%) did not return for PGIM certification.

⭕ 477 doctors left in 2022; 449 left in 2023—higher than civil war years (2006-2009).

⭕ 75 intern doctors did not apply for post-internship posts 2022-2024.

⭕ 1,209 medical officers vacated posts, ~6% of ~20,000 medical officers.

⭕ UK 🇬🇧 NHS & Community Health Service employed 3,082 🇱🇰 doctors in 2024, up 121% from 2019.

⭕ Sri Lanka ranked 4th largest source of international medical graduates in Australia 🇦🇺 (2022).

⭕ Shortage has “serious” impact on patients, caregivers, hospitals.

⭕ Psychiatry, anaesthesiology, emergency medicine, radiology, paediatrics most affected.

⭕ Policy suggestions: strict bond enforcement, dual citizenship use, bilateral tax-sharing, better working conditions, improved training infrastructure.

⭕ Urgent, evidence-based action needed to protect healthcare in 🇱🇰 and similar low-income countries.

  1. If you want to keep doctors, facilitate them. According to our osthars, doctors are not allowed for salary increments, no vehicle permits and no privileges. They must work as other officers in similar grade in gov. But when they start leaving, ane menna awurudu 13k rate salli walin igena gena rata yano. maru logic! Finally y’all will get some MBBS munna bhais from India!

  • Time to rethink and revamp the so-called free education system into a more flexible and a more autonomous system. This will help both the country and the individual. Let the financially-able pay in some way or the other and thrive. Let not-so-financially-able to get paid and thrive. Warmly, A Sri Lankan individual having all foreign higher-ed qualifications and whose parents never let the government to spend a dime on her education and higher education (because they were able to)

  • Someone mentioned age in a comment. Although it goes beyond the scope of the post , this perhaps is the perfect answer for him. :)😂 "The median age of the world's billionaires, who represent a significant portion of the wealthiest individuals, is 67. A large percentage, 42%, are over 70 years old, while less than 10% are under 50"

  • Why is it always about doctors? What about other professions? If you do the math, there are more engineers and science graduates who go abroad, and most of them never return to their homeland. Why do people always blame doctors? If some individuals propose that only doctors should be banned from migrating and put under a bond, should that apply to everyone else as well, including laborers? After all, everyone benefits from free education.

  • What about other fields such as IT, engineering, and finance? How does the standard work-life balance in Sri Lanka's healthcare system compare in terms of quality of life (QOL), even after five or more years of graduation? After completing the hardest one-year internship (24-hour on-call duty for 365 days with minimal salary) and another 4–8 years of postgraduate training, do you think the majority still stay on the minimum salary scale?

  • What about other graduates. So only the doctors are migrating. Why no one is bothering to find those stats and compare?

  • No need to force doctors to repay for their education, no need to suspend their privileges. Just allow more PMCs 😊

    3. Anuradha Priyankara Doctors in this country who get medical education for free are leaving this country because of inadequate pay among other things. A graduate from a private medical faculty would pass out after spending millions of rupees. What makes you think that those doctors would be willing to stay in this country for a meager pay?

    4. Dilshan Uthpala Hundreds of students go overseas each year for medical degrees and come back to Sri Lanka despite they are spending millions out of their pockets for the degree. There was never a shortage of candidates for local medical degrees and demand remains still very high. All the doctors in the comment section are complaining about extra work hours they put and lack of staff on government hospitals. So obviously there is a shortage of doctors even according them? Tell me what you see wrong here? And if they are willing to leave, why would you even care, they spent their own money for the degree.

    5. Anuradha Priyankara Study performed in 2017 indicates that in 2025 SL was able to fulfil no of doctors requirement. However, due to the economic downturn this was not success. So, at the current rate, this can be achieve in 2030. To address current shortage, the best solution is mandate mandatory service period after graduation with the increase benefits to retain doctors. https://www.researchgate.net/publication/322149458_How_many_doctors_should_we_train_for_Sri_Lanka_System_dynamics_modelling_for_training_needs#:~:text=Two%20sub%20models%20of%20“need,to%20be%20trained%2C%20decided%20accordingly.
